We see paperwork as a tool to help make sure that everyone is on the same page with liability, risks, and what each person is responsible for.  Some of the forms below help to evaluate if a student is medically fit before the course starts, while others help us track training and are used a continuously throughout the course.  Your instructor will reference various documents below that are required for a specific course.
